SUBJECT:    SHOOTING INCIDENT RESULTING TO DEATH

I.            Time/Date/ Place Of Incident:

1.    On or about 07:00PM o’clock in the evening of 26^th day of November 2011, along Governors Drive of Barangay San Gabriel, General Mariano Alvarez, Cavite.

II.         Victim/s:

1.) Rolando Grutas Cedro, 55 years old,

born on May 31,1956 native of magallanes sorsogon city, married

Dispatcher of Bus at DNS shuttle Service inc,

Resident of Mabuhay 2000, Barangay paliparan, Dasmariñas, City, Cavite

III.    Suspect/s:

          Unidentified malefactor/s

V.      WITNESSES:

          No witness obtain,

          Still for follow-up investigation;

1.) One Mitsubishi Coaster Bus, Bearing Plate no. PYY 857, color White and Blue,

VI.     Synopsis

                   At about 07:20PM of November 26, 2011, a Phone Call received by this station informing that an alleged Shooting Incident Transpired along Governor’s Drive of Barangay San Gabriel, this Municipality. Elements of this Station together with the duty investigator immediately proceeded at said place. Initial investigation conducted disclosed that on or about 07:00 pm of this date while the victim driving one (1) DNS Shuttle Service Coaster Bus, bearing plate number PYY-857, heading west direction of the said road when unidentified suspect/s arrived from opposite lane (in front of Española Machine Shop in Barangay, Bulihan, Silang Cavite) armed with short firearm and shot the victim six (6) times. Suspect/s fled away going to south direction (going Carmona). Victim rushed by our mobile patrol car to San Jose hospital, Barangay Gavino Maderan this municipality and later transferred to Asian Medic Family Hospital and Medical Center in Dasmariñas, Cavite. But declared dead on arrival (DOA) by attending physician Due to its six (6) shot of bullet to the body. Four (4) pcs of spent/empty shell and three (3) bullet slugs of undetermined caliber were recovered from the scene was brought to the Provincial Crime Laboratory at Imus cavite for the identification of the number of fire arm used.
